11 core recommendations for diabetes campaign

2022-03-18

Recently, the American Sports Medical Association (ACSM) released the latest expert consensus on sports/physical exercise for patients with type 2 diabetes. The consensus includes core principles, specific sports events and frequencies, as well as recommendations for specific groups of elderly patients, adolescents and patients with type 2 diabetes complications. The full text was published in the journal Medicine and Science in sports and exercise (if: 5.411).
11 core recommendations
1. regular aerobic exercise can improve blood glucose management and reduce hyperglycemia time in adults with type 2 diabetes. HbA1c decreased by 0.5% - 0.7%.
2. "High intensity resistance exercise" is more effective than "low intensity resistance exercise" in improving overall blood glucose management and reducing insulin level.
3. Compared with exercise before meals, exercise after meals (such as walking) is more recommended. Postprandial exercise can better control blood glucose by reducing the acute blood glucose peak. Regardless of the intensity or type of exercise, more energy consumption after meals can reduce blood glucose. The longer the duration (≥ 45 minutes), the more obvious the benefits.
4. Reducing sedentary time through a small amount, many times and regular exercise can moderately reduce postprandial blood glucose and insulin levels, especially for patients with insulin resistance and high body mass index.
5. Weight loss ≥ 5% (through changes in diet, exercise and lifestyle) seems to be a necessary condition for beneficial effects on HbA1c, blood lipids and blood pressure.
6. in order to reduce the visceral fat of type 2 diabetes patients, 4-5 days of moderate intensity exercise should be carried out every week (consuming about 500 kcal of heat each time).
7. for patients with type 2 diabetes, lifestyle intervention plus metformin is not superior to metformin alone in controlling blood sugar.
8. consensus suggested that adolescents with type 2 diabetes should have a consistent amount of exercise with the general population (limited data): at least 150-300 minutes of moderate intensity aerobic exercise every week, or 75-150 minutes of high-intensity aerobic exercise or combination of them. Resistance exercise shall be carried out at least 2 days a week.
9. whether or not you have diabetes, the guidelines suggest that pregnant women should have 20-30 minutes of moderate intensity exercise a day.
10. in order to prevent hypoglycemia during exercise and post exercise, it is recommended that type 2 diabetic patients who use insulin or insulin secreting agents supplement carbohydrates as required (or, if possible, reduce insulin dose).
11. Participating in the exercise program before weight loss surgery can improve the effect of surgery, and participating in the exercise program after surgery can bring additional benefits.
